Abstract Дисертація присвячена вивченню впливу поліморфних варіантів генів циклу вітаміну К (G-1639A і С1173Т гена VKORC1 та Arg406Arg гена GGCX) на розвиток ішемічного атеротромботичного інсульту (ІАТІ). Встановлено, що у представників української популяції існує зв'язок між частотою генотипів за поліморфними сайтами G-1639A (ген VKORC1) і Arg406Arg (ген GGCX) та ризиком розвитку ІАТІ. Носії мінорного А-алеля (G-1639A-поліморфізм) мають у 2,1 раза більшу ймовірність настання ішемічного інсульту, ніж гомозиготи за основним G-алелем. Зазначений ризик зростає в осіб жіночої статі, пацієнтів із надмірною вагою, артеріальною гіпертензією та без звички палити. Ризик розвитку інфаркту головного мозку в осіб з генотипом Т/Т (Arg406Argполіморфізм) y 3,1 раза вищий, ніж у носіїв основного С-алеля. Водночас гетерозиготний С/Т-генотип у жінок зменшує вірогідність розвитку ІАТІ, якщо порівнювати з генотипами С/С і Т/Т. Аналіз поєднаного впливу показав, що гаплотипи -1639G/1173T та -1639A/1173C пов’язані зі збільшенням ризику ІАТІ, а гаплотип -1639G/1173C є протективним фактором щодо його розвитку. Також виявлено, що поєднання в однієї особи артеріальної гіпертензії з носійством А-алеля за поліморфізмом G-1639A пов’язане з високим ризиком настання ІАТІ.
Диссертация посвящена изучению влияния полиморфных вариантов генов цикла витамина К (G-1639A и С1173Т гена VKORC1 и Arg406Arg гена GGCX) на развитие ишемического атеротромботического инсульта (ИАТИ). Установлено, что у представителей украинской популяции существует связь между частотой генотипов за полиморфными сайтами G-1639A (ген VKORC1) и Arg406Arg (ген GGCX) и риском развития ИАТИ. Вероятность наступления ишемического инсульта у носителей минорного А-аллеля (G-1639A-полиморфизм) в 2,1 раза выше, чем у гомозигот по основному G-аллелю. Указанный риск возрастает у лиц женского пола, пациентов с избыточным весом, артериальной гипертензией и без привычки курить. Риск развития инфаркта головного мозга у лиц с генотипом Т/Т (Arg406Argполиморфизм) в 3,1 раза выше, чем у носителей основного С-аллеля. При этом гетерозиготный С/Т-генотип у женщин уменьшает вероятность развития ИАТИ, если сравнивать с генотипами С/С и Т/Т. Анализ сочетанного воздействия показал, что гаплотипы -1639G/1173T и -1639A/1173C связаны с увеличением риска ИАТИ, а гаплотип -1639G/1173C является защитным фактором касательно его развития. Также выявлено, что сочетание у одного человека артериальной гипертензии с носительством А-аллеля за полиморфизмом G-1639A связано с высоким риском наступления ИАТИ.
Thesis is dedicated to the study of vitamin K cycle genes polymorphic variants impact on the development of ischemic atherothrombotic stroke (IAS). The study group included 170 unrelated Ukrainian patients with a mean age of 64.7 ± 0.73 years who had IAS. The control group consisted of 124 individuals with the absence of cardio-vascular pathologies. Genotyping of VKORC1 (G-1639A (rs9923231) and C1173T (rs9934438)) and GGCX (Arg406Arg (rs2592551)) gene polymorphisms was performed using PCR-RFLP (polymerase chain reaction with following restriction fragment length polymorphism analysis) method. Most statistical analyses were performed using Statistical Package for Social Science software (SPSS, version 17.0, Chicago, IL, USA). Linkage disequilibrium (LD) and haplotype frequencies were analyzed by Arlequin (version 3.1, Bern, University of Berne, Switzerland). Multifactorial dimensionality reduction was used for modelling the interactions between loci. All statistical tests were two-sided, P < 0.05 was considered significant. The data showed that G-1639A polymorphism was related to IАS, regardless of adjustment for age, sex, body mass index, smoking status and arterial hypertension. The risk for IАS in -1639A allele carriers (OR = 2.138, P = 0.015) was higher than in individuals with G/G genotype. Genotypic association of G-1639A was also revealed in women after adjustment for covariates of age, BMI, smoking status and arterial hypertension under dominant (Padj = 0.038, ORadj = 2.848, 95% CI = 1.058–7.665) and additive (Padj = 0.049, ORadj = 2.888, 95% CI = 1.006–8.293) model. In men significant difference was present only in the crude additive model (Pobs = 0.049, ORobs = 2.240, 95% CI = 1.002–5.007), but was lost after adjustment (Padj = 0.077). The association between G-1639A SNP and IAS was also found to be significant in individuals with BMI ≥ 25kg/m2 under dominant model with or without the adjustment for gender, age, smoking and arterial hypertension (Pobs = 0.008, Padj = 0.016, ORadj = 2.391, 95% CI = 1.180–4.843). The frequencies of G-1639A genotypes were different between the cases and controls with arterial hypertension either before or after adjustment for the covariates under dominant (Pobs = 0.002, Padj = 0.029, ORadj = 2.374, 95% CI = 1.091–5.166) and recessive (Pobs = 0.015, Padj = 0.049, ORadj = 2.862, 95% CI = 1.003–8.169) model. Significant association under additive model after adjusting was revealed only for A/A genotype (Padj = 0.029, ORadj = 4.029, 95% CI = 1.153–14.077). Analysis of C1173T genotypic association with IAS under the four common models of inheritance did not find the link with ischemic stroke either in total group or subgroups by gender, BMI and BP under different inheritance models. The association was absent both without and with adjustment (Pobs > 0.05, Padj > 0.05). It was also revealed that GGCX Arg406Arg polymorphism was related to IAS in Ukrainian population. The risk for this disease in patients with T/T genotype was higher than in major C-allele carriers (ORobs = 3.117; 95% CI = 1.016-9.566; Рobs = 0.047). After dividing patients into subgroups, formed by the presence of certain risk factors for atherosclerosis, similar association has been established for women and non-smokers. At the same time the heterozygous genotype (C/T) in females had significantly protective effect against ischemic stroke development when compared to C/C and T/T genotypes (Pobs = 0.048; ORobs = 0.460; 95 % CI = 0.213-0.994). Statistical significance of these results persisted even after adjustment for age, BMI, smoking and hypertension (Padj = 0.046, ORadj = 0.474, 95% CI = 0.257-0.971). Haplotype analysis demonstrated that -1639G/1173T and -1639A/1173C were related to increased risk for IAS (OR = 3.813, P = 0.010 and OR = 2.189, P = 0.011, resp.), while -1639G/1173C was a protective factor for ischemic stroke (OR = 0.548, P < 0.001). It was also found that combination of the hypertension with A-allele for the G-1639A polymorphism in one person is associated with a high risk of IAS.
