Потенціальні можливості ортопантомограм для виявлення прихованого карієсу зубів

Abstract

Карієс зубів та його ускладнення є найбільш поширеними захворюваннями порожнини рота. Основним методом виявлення каріозних порожнин є клінічний метод. Але в клініці зустрічаються каріозні порожнини, які локалізуються під вкритими протезами зубів або реставрацією, на коренях під яснами, контактних поверхнях при скупченні зубів, у глибоких фісурах з вузьким вхідним отвором. Ці ураження відносять до прихованого карієсу, вони недоступні для візуального та інструментального обстеження. Несвоєчасне виявлення прихованого карієсу призводить до виникнення ускладнень карієсу зубів, що може спричинити в подальшому розвиток запальних процесів в щелепно-лицевій ділянки і привести до втрати зубів [5]. Нагальним є питання вивчення частоти виявлення прихованого карієсу. Ця інформація необхідна для оцінки можливого покращення ранньої діагностики, яка забезпечить вчасне лікування та уникнення значного поширення каріозного ураження і розвитку тяжких ускладнень щелепно-лицевої ділянки [3].

The detection of hidden dental carious cavities with clinical methods causes some difficulties, that is why there is an urgent need to find more effective methods. Dental panoramic x-rays is an additional method of examination which provides a complete picture of dentition in one shot without a high radiation exposure. The aim of the study was to explore the possibilities of dental panoramic xrays in the diagnosis of hidden caries. Research Methods. On 32 panoramic x-rays it was analyzed the presence of hidden caries in 769 teeth depending on the sex of the patients, tooth group specificity, surface and depth of affection. Results. The hidden carious cavities were found in 12.7 ± 2.7% of teeth. Among men and women there is the same number of cavities. 64.2 ± 3.2% cavities (from the total quantity) were found in molars, 20.5 ± 3.6% in premolars, 15.3 ± 3.2% in the front group of teeth. Concerning the depth of the damage, the most often it was found the cavities of average depth (77.6 ± 4.8%). 48.0 ± 4.3% from the total quantity were of postprimary origin. In 27.5 ± 4.1% of cases the caries damaged contact surfaces, in 20.5 ± 4.0% - chewing, in 4.0 ± 3.2% - the roots of the teeth. Conclusions. It was proved the efficiency of panoramic radiographs in the diagnosis of hidden caries in teeth with different tooth group specificity, on different surfaces and different depths of damage.
