Abstract Метою роботи було встановлення взаємозв’язку між станом мікробіоценозу ротової порожнини і рівнем місцевої імунної реактивності у дітей, хворих на муковісцидоз. У ході дослідження виявлене вірогідне зростання у 2 рази активності уреази та зниження майже в 1,5 рази активності ліцозиму у слині дітей, хворих на муковісцидоз, порівняно зі здоровими дітьми. При цьому спостерігалося зниження концентрації секреторного IgA у 1,3 рази і підвищення концентрації інших імуноглобулінів, а саме IgA – у 2 рази, IgG – у 1,4 рази і IgM – у 1,5 рази у дітей, хворих на муковісцидоз, порівняно із контрольною групою. Порушення місцевого імунітету супроводжувалися зростанням дисбіозу ротової порожнини у 3 рази. У більшості хво- рих дітей відмічався незадовільний та поганий стан гігієни порожнини рота, за індексом Гріна-Вермілліона, та середній і тяжкий ступінь гінгівіту, за папілярно-маргінально-альвеолярним індексом. Максимальні порушення були зареєстровані у дітей 2–3 років.
Целью работы было установление взаимосвязи между состоянием микробиоценоза ротовой полости и уровнем местной иммунной реактивности у детей, больных муковисцидозом. В ходе исследования обнаружено достоверное возрастание в 2 раза активности уреазы и снижение почти в 1,5 раза активности лицозима в слюне детей, больных муковисцидозом, по сравнению со здоровыми детьми. При этом наблюдалось снижение концентрации секреторного IgA в 1,3 раза и повышение концентрации других иммуноглобулинов, а именно IgA – в 2 раза, IgG – в 1,4 раза и IgM – в 1,5 раза у детей, больных муковисцидозом, по сравнению с контрольной группой. Нарушение местного иммунитета сопровождались ростом дисбиоза полости рта в 3 раза. У большинства больных детей отмечалось неудовлетворительное и плохое состояние гигиены полости рта, по индексу Грина-Вермиллиона, а также средняя и тяжелая степень гингивита, по папиллярно-маргинально-альвеолярному индексу. Максимальные нарушения были зарегистрированы у детей 2–3 лет.
Actuality: The nature of the pathological changes of the oral cavity tissues and teeth in cystic fibrosis is not described sufficiently in the literature. Further study of the dependence of dental diseases development in children on primary somatic disease is necessary. Objective: Determination of the relationship between the state of microbiota of oral cavity and the level of the local immune reactivity in children with cystic fibrosis. Materials and methods: For this goal 41 children aged 2 to 17 years were examined. The basic group included children with cystic fibrosis (n = 23) who were diagnosed with periodontal and tooth disease, and group of control consisted of their coevals without concomitant somatic pathology and who had not cystic fibrosis (n = 18). The survey included patients' medical history, clinical examination of the oral cavity, definition of the Green-Vermillion hygiene index and gingivitis index (PMA), urease activity by the reaction of urea to form ammonia and lysozyme by the bacteriological method using a substrate suspension Micrococcus lysodeicticus. Degree of dysbiosis of oral cavity was determined by enzymatic method after A. P. Levitsky and others by correlation of the relative activity of urease and lysozyme. For evaluation of local immunity state the levels of immunoglobulins IgA, IgM, IgG and secretory immunoglobulin sIgA were determined by ELISA. Results and discussion: The study found significant increase of urease activity in 2 times and reduction of lysozyme activity almost in 1.5 times in the saliva of children with cystic fibrosis compared with the healthy children. While there were decrease of secretory IgA concentration in 1.3 times and increase of concentrations of other immunoglobulins such as IgA – 2 times, IgG – 1.4 times and IgM – 1.5 times in children with cystic fibrosis compared to the group of control. Disorders of local immunity were accompanied by the growth of oral dysbiosis by 3 times. Most of the sick children were marked unsatisfactory and poor state of oral hygiene due to GreenVermillion index, moderate and severe degree of gingivitis after the papillary-marginally-alveolar index. The maximum violations had been registered in children at the age of 2 to 3. Conclusions: Low level of oral hygiene of children with cystic fibrosis is accompanied by significant increasing of oral cavity dysbiosis degree, which indicates a damage of microbiota caused by first of all decrease of antimicrobial protection.
