Abstract У статті представлені результати дослідження особливості внутрішньоклітинного розподілу йоду в крові дітей з позалікарняної пневмонією різного ступеня тяжкості на тлі йододефіциту. У крові досліджуваних визначали вміст загального, органіфікованого і неорганічного йоду. Для оцінки йодної забезпеченості використовували показник екскреції йоду з сечею. Результати оцінювали в залежності від тяжкості пневмонії і порівнювали з показниками групи здорових дітей. У всіх досліджуваних групах виявлено супутній йододефіцит різного ступеня тяжкості. Встановлено зниження концентрації загального та органіфікованого йоду і збільшення вмісту неорганічного йоду щодо групи контролю. Виявлені показники внутрішньоклітинного пулу йоду змінювалися щодо ступеня тяжкості захворювання. У ході проробленої роботи доведено залежність між тяжкістю захворювання і «неправильним» внутрішньоклітинним розподілом йоду на фракції на тлі виявленого йододефіциту. Таким чином, супутній йододефіцит призводить до різкого підвищення концентрації неорганічного йоду з одночасним зниженням рівня органічного і загального йоду в крові, що призводить до більш важкого перебігу пневмонії і розвитку ускладнень.
В статье представлены результаты исследования особенности внутриклеточного распределения йода в крови детей с внебольничной пневмонией разной степени тяжести на фоне йододефицита. В крови пациентов определяли содержание общего, органифицированного и неорганического йода. Для оценки йодной обеспеченности использовали показатель экскреции йода с мочой. Результаты оценивали в зависимости от тяжести пневмонии и сравнивали с показателями группы здоровых детей. Во всех исследуемых группах выявлен сопутствующий йододефицит разной степени тяжести. Установлено снижение концентрации общего и органифицированного йода и увеличение содержания неорганического йода относительно группы контроля. Выявленные показатели внутриклеточного пула йода изменялись относительно степени тяжести заболевания. В ходе проделанной работы доказана зависимость между тяжестью заболевания и «неправильным» внутриклеточным распределением йода на фракции на фоне выявленного йододефицита. Таким образом, сопутствующий йододефицит приводит к резкому повышению концентрации неорганического йода с одновременным снижением уровня органического и общего йода в крови, что приводит к более тяжелому течению пневмонии и развитию осложнений.
Introduction. Community-acquired pneumonia (CAP) is a leading cause of morbidity and mortality globally. The severity of the course of CAP in childhood depends on difficulties with concomitant pathology, the state of the endocrine system and the immunity of the organism, especially among people living in conditions of endemic iodine deficiency. The aim of the study was to study the features of intracellular distribution of iodine in the blood of children with community-acquired pneumonia (CAP) living in iodine deficiency areas. Materials and methods. We investigated 74 children with CAP aged 4–14 years (45 children with moderate and 29 with severe pneumonia) and 35 healthy children. The severity of pneumonia in children was assessed according to the classification criteria of the disease. All children were assessed the level of iodine in the urine; inorganic iodine, total and organificated iodine in the blood. Results. According to iodine median we detected mild iodine deficiency in all study groups (36.2 μg/L, 63.8 μg/L and 61.9 μg/L, respectively (p ˂ 0.05)). When examining the intracellular pool of iodine of patients from group I showed an inverse relationship between the levels of iodine distribution for organic and inorganic iodine (r = -0.737, p < 0.05), which tends to decrease depending on the severity of pneumonia. A close correlation between the levels of total and organic iodine (r = 0.538, p < 0.05) Data of group II patient indicate a slight decrease in the level of total and organificated iodine, as well as a moderate increase in the concentration of inorganic iodine in the blood of patients. These changes are less pronounced when compared with the results of group I, but statistically significant (p < 0.05). It has been established not significant reduction in organic and total iodine and not much increase in the level of inorganic form compared with the control (p ˂ 0.05). When carrying out the correlation analysis among Group I patients, we proved a close соrelation between the changes in the intracellular pool of iodine and the clinical and laboratory data characterizing the severity of the pathological process in pneumonia. In particular, a close relationship was found between the concentration of inorganic iodine and the sum of scores on the Pneumonia Severity Index (r = 0.783; p < 0.05). These data confirm the effect of severe pneumonia on changes in intracellular distribution of iodine in children. Conclusions. The revealed changes in the indices of the intracellular pool of iodine and its distribution in the child's body are directly proportional to the severity of CAP. Severe course of СAP in children could be caused by disruption of iodine organification in the body. Reduction in organic and total iodine and an increase in the level of inorganic form during persistent iodine deficiency lead to more severe course of pneumonia in children.
