    Tackling correption in the health sector

    Corruption in the health sector is unique because it includes abuse by public officials and unscrupulous behavior by other actors (drug manufacturers, health professionals, patients, etc.). The urgency of solving the scientific problem lies in the fact that financial relations in the medical field arise between a wide range of persons who can act as initiators of corruption decisions and/or their executors. The study’s main goal is to analyze the forms of corruption in the healthcare system and their consequences for society, as well as to assess the degree of interrelationship between the level of corruption and indicators of financial support in the healthcare sector. The methods of structural and comparative bibliometric analysis and correlation analysis became the methodical tools of the conducted research. The object of research is 140 countries in the world. The conducted bibliometric analysis testified to the permanent growth of the study of the issue of corruption in medicine in the scientific environment. Scientists from the United States, United Kingdom, and Germany have implemented the largest number of works on a certain topic. The conducted correlation analysis also empirically confirmed that an increase in the level of corruption in the country leads to an increase in the share of household expenses for financing their own medical needs. The article founds that the population in countries with a low level of corruption (the corruption promotion index is closer to 100) has a higher level of provision of vital medical services (reproductive health, health of women, newborns, and children, infectious diseases, non-infectious diseases and opportunities provision of services and access to them). Countries with a high level of anti-corruption and coverage of medical services include Finland, Sweden, Germany, and Great Britain. The results of the research can be useful for state regulatory bodies, specialized organizations in the field of health care, as well as public organizations.

    The article discusses approaches to optimizing the health care system in the context of reform. In the article identified institutional and organizational constraints to shape the health care system model with the necessary components: quality assurance, financial constraints, and optimization of budgetary resources. In the article developed the approaches to the formation of the health care system at the regional level. To this aim, the factors that influence the efficiency of the organization and functioning of the medical system are identified. Particular attention is paid to the organization of quality medical care in the united territorial communities of the Sumy region. The factorial analysis made it possible to form models of organization of medical institutions in the united territorial communities, taking into account financial and organizational constraints. In the process of research used the conditionally constant and variable factors in the model to adapt effectively to changes in the external environment. Modeling is based on a functional approach (model based on data flow construction). This approach allows you to identify functional problems of the system, to determine the basic requirements of stakeholders, to determine the structure of information flows in the model. In this case, the health care system is considered as a set of processes (related or independent) that achieve this goal. Then the overall management of the system can be represented as the management of a set of these processes in order to obtain the end result (with the specified characteristics). The peculiarity of this approach to modeling is to summarize a large array of input information, to establish direct and feedback between economic, demographic, geographical, social indicators of community development and medical indicators (results) that characterize the level of public health. The introduction of an integral indicator in the calculation model will solve both general institutional tasks, such as temporal dynamics within the created unified territorial community and creating a competitive position in comparison with other unified territorial communities of the region, and making local management decisions based on the influence of individual factors on the functioning of the health care system. The application of the model makes the practical importance in the control of deviation of the actual value of the integral indicator from the normative and planned result.

    Analysis of Trends in the Development of Availability of Medical Services for Rural Population

    The article summarizes the arguments and counter-arguments within the scientific discussion on the availability of medical services for the rural population. The main purpose of the study is to analyze the current principles of state policy of Ukraine in the field of health and accessibility and quality of health care in rural areas. Systematization of literature sources and approaches to solving the problem of access to health services has shown that many problematic issues in the field of access to health services in rural areas in Ukraine and around the world, related to investment, personnel management and effective government regulation. The urgency of solving this scientific problem is that medical care should be provided in all regions and agglomerations of the country equally effectively and focus on patient requests. The study of the availability of medical care in rural areas in the article is carried out in the following logical sequence: bibliometric analysis of research, the next step is a survey of consumers of medical services on the basis of the district hospital; analysis of the results of the survey of respondents; providing guidance on key health issues and areas. The methodological tools of the study were survey methods and expert assessments, comparative and bibliometric analysis, the study period was chosen 2011-2021. The object of the study was the Sumy Central District Clinical Hospital, as it provides services to patients living in rural areas. The study empirically confirms and theoretically proves that there are problems with having a family doctor in families, as 1% of respondents indicated that they do not have a family doctor and have not consulted him, while more than half of respondents are satisfied with the result of a doctor’s visit, however, a significant share did not indicate the exact answer, which gives reason to think about the existing problems that led to such results. The results of the study can be useful for further research and recommendations for future opportunities to effectively influence the development of access to health services in rural areas and at the same time improve and enhance the quality of services provided in cities.

    This paper summarizes the arguments and counterarguments within the scientific discussion on the issue open science in health economics in the reality of the Covid-19 pandemic. The aim is to study the openness of the results of scientific work in the field of health economics. To achieve this goal, the following tasks have been set: to conduct a bibliometric analysis of the concepts of open science, health, health economics, research, education; author’s structuring of the concept of open science in the context of health economics; identification of the main stakeholders and their role in the field of open science in the field of health economics. The object of research is the field of health economics. The subject of research is the determinants of the impact of open science on the health economy. In order to identify the vectors of scientific developments in recent years in the field of open science and health economics, the method of bibliometric analysis was used using the tools of the Scopus database and the software VOSviewer v.1.6.10. Based on the selected works, terminological maps were constructed for individual categories and the main clusters were identified, as a result of which the intensity of the use of one term with others was assessed. Scientific discoveries play an important role in health care. Especially in a pandemic, access to up-to-date data will save more lives and preserve the health of the world’s population. After all, the level of economic well-being of a country depends on the level of health of the population and its ability to work. It is determined that open science should include combining knowledge and efforts to support research in the field of health economics and reduce the knowledge gap between countries; mobilizing decision-makers, researchers, innovators, publishers and civil society representatives to ensure free access to scientific data, research results, educational resources and research facilities in the field of health economics; strengthening the links between science and political decisions to meet societal needs; ensuring open science for society, despite the fact that the borders between countries are closed. The results of the study can be useful for researchers who have been conducting study on this topic, and students majoring in ‘Health Economics’.

    Health Care in Rural Areas in Ukraine: Current Business Processes and Prospects

    The pandemic period showed uneven access of the population to quality medical care. The rural population was the most vulnerable. Accordingly, the study’s primary purpose is to determine the level of satisfaction of the rural population with the quality and accessibility of medical services (for example, the rural population of Sumy City Council), establish сausal link, and develop recommendations for improving health care in rural areas.

    This article summarizes the arguments and counterarguments within the scientific discussion on health care financing by introducing compulsory health insurance in Ukraine and improving voluntary one. The study’s primary purpose is to theorize and determine the main directions and recommendations for implementing compulsory health insurance in Ukraine. Systematization of scientific background concerning the study problem showed that Ukrainian national and municipal health care institutions didn’t provide free medical care in the amount and quality guaranteed by the Constitution of Ukraine. Therefore, the relevance of solving this scientific issue is the necessity of comprehensive health care reforming (especially funding) considering compulsory health insurance.

    This article summarizes the arguments and counter-arguments within the scientific discussion on personnel management in health care organizations. The study’s main purpose is to identify opportunities to improve the efficiency of medical staff through innovative forms of labor organization and HR management. Systematization of scientific background and approaches on personnel management in health care showed that staff development is a priority in health care. The authors emphasized the high competition in the Ukrainian services market. Thus, improving the service quality is the key element for successful any business activity.

    This article summarizes the existing approaches to investigating instruments of responsible investments in the health care system in Europe and in United States. The main research’s purpose is to identify existing instruments of responsible investment under funding Sustainable Development Goal 3: ‘Good health and well-being’. Systematization of scientific sources and approaches on the investigated issue showed no unique approach to forming a list of responsible investment instruments to finance health and well-being in Europe and United States. Hence, existing approaches vary by risk, return, suitability for financing, and so on.
